1) To give you an idea of the
qualifications required of the nurse applicant as well as the compensation and
benefits we provide to selected candidates, start by
downloading this application form that you will be required to fill
out and sign where appropriate.
2) When completed, please send
it to us by express mail along with the copies of additional requirements, i.e.
resume, license, diploma, employment authorization, CGFNS certificate,
transcript of records, marriage certificate (if applicable), employment
certificates, passport, (3)-2”x2” passport photos and valid CPR certificate
(optional).
3)
Upon validation of your
credentials, we will then send you the Employment Contract along with additional
forms required for your employment in the US.
